HCBS Program Coordinator - Full Time

The job function of the HCBS Coordinator is to provide daily supervisory management and leadership of the HCBS Program.

Ensure that all day-to-day issues and requirements are complete, and effectively address and resolve all

concerns within the program, as well as carry an HCBS caseload of members and providers.

Office

RESPITE (RSP) - $17 an hour

The Direct Care Worker providing Respite will provide short-term care and supervision consistent with the health needs of an individual with a developmental disability. Services are provided as a planned or unplanned non-routine interval of rest and/or relief to a family member or other unpaid person who resides with and provides ongoing care for a consumer. The Direct Care Worker will deliver services with compassion and respect.

ATTENDANT CARE (ATC) - Starting at $17.50 an hour

The Direct Care Worker providing Attendant Care will provide services in order for an individual with a developmental disability to remain in her/her home and/or participate in work/community activities. The Direct Care Worker will assist the consumer to attain or maintain safe and sanitary living conditions and/or maintaining personal cleanliness and activities of daily living. The Direct Care Worker will deliver services with compassion and respect.

HABILITATION (HAH) - Starting at $19 an hour

The Direct Care Worker providing Habilitation will provide a variety of interventions designed to maximize the functioning of a person with a developmental disability. Services are designed to assist individuals in acquiring, retaining, and improving self-help, socialization, and adaptive skills necessary to reside successfully in home and community-based settings. The services include the provision of training in independent living skills or special developmental skills, orientation and mobility training, sensorimotor development, and behavioral management.

JOB COACHES (GSE) - Starting at $18-$20 per hour

In our group-supported employment program, individuals learn important functional job skills such as customer service skills, money skills, professionalism, and so much more. They will gain the skills needed to have mindful and meaningful employment for work longevity.

 

Fostering Hope provides group-supported employment services for adults with all levels of abilities. The pride of earning a paycheck is something most people want in life, we all deserve the opportunity to earn a paycheck. Earning your own money leads to confidence, pride, and a desire to want to be your own person and be self-sufficient. Fostering Hope will do everything we can to assist every person in our work program to be successful with functional work skills.

The direct support professional will be responsible for transporting our members to and from our GSE and DTA programs.
